Claude Session URL Appended to Commit Messages and PR Descriptions by Default

Claude Code appends its session URL to git commit messages and GitHub PR descriptions by default. A GitHub issue (#66504) raised this as a privacy concern: every commit made with Claude Code leaks a session identifier to repository history.
